GLP-1 Weight Loss and Lean Mass: Make Muscle Part of the Plan

Weight loss changes the number on the scale. Strength is what helps you use the body you are building. If you use a GLP-1 medication, there is another number worth discussing with your care team: how much lean mass you retain as your weight changes.

What the 2026 research actually found

A 2026 systematic review and meta-analysis pooled 20 randomized trials with 15,782 participants. Lean mass accounted for an estimated 35.2% of total weight lost with semaglutide and 25.4% with tirzepatide. The pooled lifestyle interventions that included resistance training showed a lean-mass share of 17.5%. Read the published study.

These figures describe different groups of studies. They do not show that adding weights to a GLP-1 routine cuts muscle loss in half. Lean mass also includes more than skeletal muscle. The actionable message is still clear: when weight is coming down, give strength, movement and adequate nutrition a place in the plan.
